CINCINNATI. A 29-year-old temporary worker working the night sanitation shift at an Ohio food plant was injured after just nine months on the job when he fell into an industrial mixer he was cleaning and got caught in the rotating blade. In response to an employer injury report dated October 12, 2022, an OSHA investigation found that the host employer, Zwanenberg Food Group USA Inc., did not train sanitation workers to block equipment prior to cleaning, exposing them to moving machine parts. OSHA cited a similar breach at the facility less than two weeks prior to the injury.
OSHA proposed a $1.9 million fine after citing 11 willful violations, four major violations, one repeat violation, and one non-serious violation, most of which are related to required machine safety programs that isolate energy to prevent movement during cleanup. and service. In 2017, the agency added Zwanenberg to its Serious Violations program.
Trip hazards, electrical safety procedures, lack of eye protection and personal protective equipment were also noted.
The U.S. Occupational Safety and Health Administration’s (OSHA) Regional Targeted Program for food manufacturing specifically addresses the common hazards encountered in these plants.
OSHA cited the Zwanenberg Food Group USA Machine Safety Program and other hazard violations in 2017 and September 30, 2022. The company disputed the latter violation.
